Two Mechanisms, One Injection

Cingal is a single pre-filled syringe containing two components:

  • Cross-linked hyaluronic acid (HA) — the same viscosupplementation that gives your knee mechanical cushioning and lubrication, with an expected effect duration of 6+ months.
  • Triamcinolone hexacetonide — a long-acting corticosteroid that reduces synovial inflammation quickly, giving pain relief within days.

The clinical case for Cingal is straightforward: patients with knee osteoarthritis who are also in an inflammatory flare (warmth, effusion, night pain) benefit from both an immediate anti-inflammatory effect and a longer-term cushioning effect. Cingal delivers both in one visit and one needle stick — instead of a cortisone injection now followed by a separate HA injection weeks later.

Who Cingal Is For

Good candidates

  • Knee osteoarthritis (Kellgren-Lawrence grades 1–3) with an active flare — warm knee, effusion, morning stiffness > 30 minutes
  • Patients who need rapid relief for an upcoming event (travel, work commitment, physical rehab milestone) plus a longer-term cushioning effect
  • Patients who've previously had a good response to HA but need faster onset this time
  • Shoulder OA with flare — off-label but common; discussed case by case

Less suitable

  • Poorly controlled diabetes — the steroid component can spike blood sugar for several days
  • Active joint infection — always excluded before any joint injection
  • Very early or very late-stage OA — the cost is only justified when both mechanisms are needed
  • Patients specifically wanting a steroid-free regenerative approach — PRP or straight HA is a better fit

What to Expect on the Day

  1. Assessment. The clinician confirms the diagnosis, checks whether Cingal or a different injection would give you the best result, and reviews any imaging you've had.
  2. Consent. We explain the product, expected onset, expected duration, side-effect profile and blood-sugar precautions if you're diabetic.
  3. Injection (5 min). Skin sterilised, joint identified, Cingal delivered under sterile technique. Ultrasound guidance is used where anatomically indicated.
  4. Post-injection. Rest the knee from heavy loading for 48 hours. Light walking is fine. Diabetic patients: monitor blood glucose for 5 days.
  5. Follow-up. A short check-in message at 1 and 6 weeks to confirm the injection is working.

A note from our clinicians

Cingal is a useful tool when the situation calls for it — an inflammatory OA knee that needs immediate anti-inflammatory action plus a longer-term cushion. It is not a first-line injection for a young, non-inflamed knee (PRP or straight HA does the job better there), and it is not the right choice for a very advanced grade-4 knee (surgical opinion first). We spend the assessment time to be sure Cingal is right for you before we recommend it.
