GLP-1 receptor agonists like Wegovy (semaglutide) and Mounjaro (tirzepatide) are highly effective medications for weight loss — but the medication alone is only part of the picture. Sustainable results come from combining the injection with a proper medical assessment, nutrition guidance, activity progression and regular follow-up.

At Inflexion we treat weight management as a medical condition: full history, physical examination, baseline labs (fasting glucose, HbA1c, lipid panel, liver and kidney function, thyroid), cardiovascular risk assessment, and a shared decision about whether GLP-1 therapy is the right fit — and if so, which drug.

Wegovy (Semaglutide)

Once-weekly subcutaneous injection. Approved for chronic weight management in adults with a BMI of ≥ 30, or ≥ 27 with weight-related comorbidities.

How it works

  • Mimics the gut hormone GLP-1, slowing gastric emptying and reducing appetite via central pathways.
  • Typical weight loss in clinical trials: ~15% of body weight at 68 weeks alongside lifestyle intervention (STEP programme).
  • Dose titrated weekly over 16 weeks to the maintenance dose to minimise nausea and GI side effects.

Considerations

  • Common early side effects: nausea, reflux, constipation — usually subside within 4–8 weeks.
  • Not suitable in personal or family history of medullary thyroid carcinoma or MEN 2.
  • Caution in pancreatitis history and gallbladder disease.
  • Contraindicated in pregnancy and breastfeeding — reliable contraception is required.

Mounjaro (Tirzepatide)

Once-weekly subcutaneous injection. A dual GLP-1 and GIP receptor agonist, giving greater average weight loss than semaglutide in head-to-head trials.

How it works

  • Activates both GLP-1 and GIP receptors — the dual mechanism gives stronger appetite suppression and glycaemic control.
  • Typical weight loss in clinical trials: ~20–22% of body weight at 72 weeks alongside lifestyle intervention (SURMOUNT-1).
  • Dose titrated monthly over 20 weeks — more gradual to minimise GI side effects.

Considerations

  • Common early side effects mirror semaglutide: nausea, reflux, GI upset. Slower titration helps.
  • Same thyroid, pancreatitis, gallbladder and pregnancy precautions as Wegovy.
  • May reduce effectiveness of hormonal contraception in the pill form — alternative contraception advised.
  • In head-to-head trials, tirzepatide has produced the largest average weight loss among licensed pharmacological options — but not everyone tolerates it.

Weekly or fortnightly check-ins during dose titration to manage side effects, adjust the schedule and support the lifestyle plan.

Monthly follow-ups on the maintenance dose. Weight, waist and lab trends reviewed. Nutrition and exercise plans updated with our in-house physiotherapy team.

Exit + relapse prevention

Structured taper and a maintenance plan to protect against weight regain when the medication is stepped down — the phase most weight-loss programmes underinvest in.

When GLP-1 Therapy Isn't the Right Fit

  • BMI in the normal range without a weight-related comorbidity — GLP-1 agonists are not licensed for cosmetic use.
  • Personal or family history of medullary thyroid carcinoma or MEN 2 syndrome.
  • Pregnancy, breastfeeding or planning pregnancy in the next 2 months.
  • Active severe gastroparesis, active pancreatitis or gallbladder disease.
  • Type 1 diabetes — GLP-1 agonists are not first-line and require specialist oversight.
  • Serious eating disorders — we'd refer for specialist psychological input first.
